Required Documents
Prior to filing, the Law Office must receive the following documents:
- Pay stubs for the last six months starting backwards with the first of the month in which you are filing. For example, if you file on July 15 we will need all pay vouchers from January 1 through July 15.
- If you do not have your pay stubs ask your employer for a print out showing each pay period. The printout must show the gross pay, net pay and all deductions for each pay period.
- If you are unemployed this is not required. However, i you are drawing unemployment we must have proof of your weekly allowance.
- Proof of social security payment amount. (If you do not receive social security or social security disability this is not required.) A letter from the government or a bank account statement showing the deposit will meet this requirement.
- A copy of the last two tax returns you filed. (If you have not filed a tax return in the last 5 years this is not required.) If you do not have your tax returns ask us to get you a tax transcript from the IRS.
- A copy of your state identification card or driver's license.
- A copy of your social security card.
- A copy of your bank account statement.
- After we file we will also need a copy of the bank statement showing the date of filing.
- A copy of your real estate tax bill with the fair market value of your home. (If you do not own real estate this is not required.)
- A copy of your last mortgage statement. (If you are not purchasing real estate this is not required.)
- Certificate of completion of credit counseling.
- Signed bankruptcy papers
